Payment Policy
All visits to ICT Eye, LLC will be classified as specialty appointments. Most insurance company require referrals from a primary care physician. These must be done before scheduling your first office appointment. It will be the patient’s responsibility to contact your insurance company for referral requirements as well as your primary care physician before every appointment.
Our office and the insurance companies recognize that without a required referral, the patient will be responsible for any charges related to services provided. The patient will be billed directly for any charges not covered by referrals.

Please call your pharmacy when you need medication refills. Please have your pharmacist call at least 24 hours in advance so that you will have your prescription when you need it.
